 We report cases of accident on exercise in Japan and contrive the preventive measures. According to the data from the Japanese association of safety assurance in sports, frequencies of trauma during exercise were 22,634 in 1973 and 1974; fractures 34.7%, sprains 29%, contusins 14.3%, wounds 5.4% ruptures of tendon 3.9%, dislocations of joint 2.7% and others 3.6%. Frequent medical accidents were drowing, heat strokes, submarine sickness, accidents due to cold and intrinsic acute death. Causes of 1749 acute deaths during exercise under the control of school for 9 years from 1970 to 1978 were cardiac disease 40.4%, head injury 30.2%, drowing 12.9%, cerebral bleeding 5.3%, cervical spinal injury 4.0%, visceral injury 2.7%, asphyxiation 1.7%, heat stroke 1.3%, electric shock 0.8%, and traumatic shock 0.7%. In autopsy, 50% of intrinsic acute death on exercise were status thymico-lymphatics. Hypoplasia of adrenal gland and hypertrophy of thymus were observed frequently too. To prevent the accident on exercise, a medical preexamination is important. 201 thallium myocardial scintigraphy was useful to examine the false positive cases in stress ECG because 201 thallium scintigraphy has 100% specificity.
